KUALA LUMPUR, Dec 1 (Bernama) -- Employees Provident Fund (EPF) members who wish to maintain the employee's monthly contribution rate at 11 percent from January 2009 may obtain the forms beginning Monday.
In a statement here, EPF public relations senior manager, Nik Affendi Jaafar, said the forms, KWSP 17A (AHL) Khas, were available at all EPF branches or could be downloaded from myEPF website at www.kwsp.gov.my.
"Fully completed forms can be submitted to the members' respective employers for onward submission to the EPF," he said. On the reduction in the statutory contribution rate which would take effect from January 2009 to December 2010, Nik Affendi said a new monthly contributions schedule has been issued.
"Members and employers may download the new schedule from our website or to get it from our branches natiowide today," he said. -- BERNAMA
